You need to communicate with a counselor or advocate at the current school you are to help you start the transfer process. Your school counselor should have the right contacts for you to contact at new school for Admissions requirements.
The new school you are transferring to is going to need your official school transcripts.

    Transfer Applicants
    I. Guidelines for Transfer Applicants for SY 0809 (Local and Foreign)
    A. PRE-REQUISITE
The applicant must be taking the full load requirement per semester and must have finished one full school year in the university he/she is currently enrolled in by the time the requirements are submitted.
    B. APPLICATION PROCESS
    1. Get an application form from the Office of Admission and Aid, Kostka Hall within June 18, 2007 to December 15, 2007.
    2. Register for the ACET on November 19 to December 15, 2007 (submit the exam permit only).
(Transfer applicants from the provincial schools may register for the ACET at the Provincial Testing Centers in Cagayan de Oro, Cebu, Davao, Iloilo, Naga, and Zamboanga.)
    3. Take the Ateneo College Entrance Test (ACET) on January 20, 2008 (Sunday) for Metro Manila test site and January 19, 2008 (Saturday) for Provincial Testing Centers.
    4. Submit completed forms not later than April 18, 2008 (Friday).
    a. Application form for Admission
    b. Two recommendation forms
    c. An Unofficial Transcript of college grades covering up to the last completed semester and a copy of grades of the current semester (may submit a photocopy of registration card and class cards). Please bring original copies for verification.
Note: Do not apply for Honorable Dismissal until decisions on applications have been released.
    d. An essay about the topic indicated in the application form.
    e. A photocopy of SAT I result (for applicants studying abroad who will
    not take the ACET)
    5. Those applying for scholarship must ask for a SCHOLARSHIP / FINANCIAL
    AID QUESTIONNAIRE.
    6. Decision letters will be mailed directly to the applicant’s home address.
    7. Foreign transfer applicants must make sure to read up on our
    foreign guidelines.
INCOMPLETE APPLICATION FORMS WILL NOT BE PROCESSED.
Note: A small number of transfer students is accepted into the Loyola Schools. If accepted, there is no guarantee as to the number of courses that will be validated from the previous school. Usually only one-third of the courses taken in the previous school are credited at the Ateneo. These subjects are credited upon passing the validation tests given by the departments concerned. Also, because of the structure of the Ateneo curriculum, transferees become “irregular” students. (Transferees who finish two years in another college generally need at least three more years at the Ateneo to finish their degree).
    II. DOWNLOADABLE APPLICATION FORMS
The application form for FOREIGN TRANSFER APPLICANTS for SY 2008-2009 is available for downloadhttp://www.admu.edu.ph/ateneo/www/UserFi…

Look at that school’s requirements for transfers. You’ll need your transcripts for sure so you should talk to your counselor at your current school. What school are you attending in Baguio?
